The Ohio School Immunization Level Assessment reporting period is now closed for the 2023-2024 school year.
Ohio Revised Code (ORC) 3313.67 requires all schools to report an annual summary of the immunization status of pupils by Oct. 15th to the Ohio Department of Health (ODH). Reporting for the 2023-2024 school year was to be completed and submitted to ODH by Oct. 15, 2023.
